Data must be entered by the employee in all fields at the top of Section 1. If the employee does not enter info in optional fields, for example “Apt Number” or “Other Last Names”, you should require the employee to write “N/A”. (The email address and phone number are also optional fields.)
The employee must check the first box in shaded “Preparer and/or Translator Certification” area below Section 1 on page 1, to indicate that the employee completed Section 1 on his/her own.
The expiration date of a Social Security card is “N/A”. The expiration date field should not be left blank.
